CalAIM Policy Summaries
This policy work has been supported and maintained through the development and implementation of a Community Health Worker (CHW) training and employment program managed under El Sol Neighborhood Educational Center, which will support the integration of highly qualified CHWs into care teams to provide outreach tailored to the needs of the diverse patient population with Health Related Social Needs (HRSNs). Criteria for inclusion will include emergency department visits and high HRNs (including food and housing insecurity, transportation, low education levels, and chronic school absenteeism.) located at Arrowhead Grove and residents living in the surrounding area. CHWs will connect those with high HRNs residing in and around Arrowhead Grove to a wide range of resources and services aimed at addressing social and health needs that impact health and safety through navigation service in association with the local health care system.
CHWs will aid in the execution of Community Supports by helping residents locate a medical provider, offering housing and homelessness assistance, making referrals to local health education classes, locating food, enrolling the uninsured or underinsured in Medi-Cal enrollment, and providing assistance with DMV waivers, etc. The anticipated impact of The Plan’s focus on Community Supports include : (1) Increased access to health care and reduced barrier to services, (2) Increased prevention and treatment of chronic diseases, (3) Increased access to community based services, including housing options, (4) Increased availability and access to preventative care services, (5) Increased access to needed services and resources through collaboration with community partners, (6) Formation of trusted community relationships yielding amplified community voices and community-informed priorities
CalAIM Policy Priorities
Our Accountable Community for Health is focused on the State of California’s CalAIM (Advancing and Innovating Medi-Cal) initiative to increase Community Supports for those living in and around Arrowhead Grove. Our policy priorities include:
- Identifying and collectively addressing policy and implementation gaps of CalAIM’s initiative, specifically community supports and nonclinical providers
- Convening Managed Care Providers (MCPs) or Health Plans to discuss challenges and possible solutions, particularly as it relates to housing and workforce development for CHW service providers
- Hosting Data and Research town hall presentations for community and Uplift San Bernardino ACH members
- Curating data/information sources related to the priority health issue or condition and identify data gaps
Our collaborative team, composed of nonprofits and public entities, is focusing on these needs by developing strategies that address the Vital Conditions for Health and Wellbeing in San Bernardino associated with access to housing and jobs as community health workers.
