Our Network

The role of our Accountable Community for Health is to partner with our local neighbors to establish and implement systematic roadmaps for healthy and thriving communities in San Bernardino. Our Collaborative is an exercise in civic engagement with community stakeholders. We are shaping and supporting innovative partnerships, policies, development, and programs that address community needs through the Vital Conditions for Health and Wellbeing framework. Our Collaborative’s role is to:

• Inform the development process for the Arrowhead Grove Community Resource Center.
• Ground the Community Health Worker (CHW) model of peer-to-peer empowerment to improve community health outcomes 
• To identify and collectively address policy gaps in the CalAIM program, specifically Community Supports
• To advance affordable homeownership, workforce development and entrepreneurship in the surrounding community that is locally owned and operated/occupied
• Be a strong and credible advocate for community interests.
• Secure community benefits for current and future residents (e.g. local hire, affordable housing, health services, etc.).
• Build a focus-driven and community-led Collaborative of public, private, and community partners.
• Build community leadership and capacity.
• Meet regularly and participate in workgroups and community meetings to provide leadership and guidance, promote shared learning, and collaboration.
• Meaningfully engage community voices and interests.
