In an effort to better serve the community, the Making Hope Happen Foundation has joined Unite California. Unite Us is a coordinated care network of health and social service providers.
Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities.
The platform will help San Bernardino residents connect to community programs that support residents. Residents can request a variety of services and resources offered through the platform including COVID-19 support, connections to food resources, programs for older adults and referrals to other resources. Residents also can receive information about support services.
Locally-based nonprofit organizations or resource-focused agencies are invited to join the Unite Us platform to help strengthen the resource network for City residents.
